Care Management Unit

Care Management Unit provides individualized case management services for children and adults with developmental disabilities.

Our case managers locate, manage, coordinate and monitor services and supports to preserve our clients' health, safety, and enable them to live in the community.
Services Offered
  • Service coordination and consultation.

  • Support for individuals with Traumatic Brain Injury and their families.

  • Information regarding prevention of sexual abuse and general information about sexuality issues.

  • Information and referral.

  • Assistance with guardianship and protective placements.

  • Counseling.

  • Placements.

  • In-home support for families who have children with severe disabilities.

  • Limited funding to buy needed services that cannot be bought through other sources.

  • Help in linking families with other families to strengthen natural supports.

  • Assistance and support with: nutrition, safety, discipline, budgeting, self-esteem, age appropriate play and expectations.
Who Is Eligible
  • Children and adults who are residents of Rock County and have a developmental disability or traumatic brain injury.

  • Rock County Developmental Disabilities Board and Rock County Human Services determines eligibility for services. Medical Assistance is billed for eligible services.
